Participant Agreement Form DUE TODAY

Today is the LAST day to turn in your Participant Agreement Form. It is available for download from the list of Current Documents to the right. You can scan and e-mail it in to youthprograms@cincymuseum.org, you can send it via fax to 287-7096, or you can bring it with you to the mandatory Omni meeting this afternoon. Again, today is the LAST day to turn in this form. If it is not turned in today, we will evaluate your future with the program.

Parents and Guardians - Please do not forget to complete the Updated Family Information sheet by today. Also, there will be a Parent/Guardian Advisory Board meeting today from 5 to 6.
