Current Parents/Guardians - Updated 2014 Family Information

As you all know, the majority of Youth Programs events are funded through grants and private donations.  Each youth’s family has completed a questionnaire regarding family information upon entering the program and this is used to receive grant funding.  Please fill out our new information sheet so we can provide updated information for the current year as government income qualifications change every year. We have made a survey available via Survey Monkey to help make this process easier. If you are uncomfortable with filling out an online survey, we will have hard copies available at Museum Center or we will be happy to send you the document via e-mail that you can print and fill out.
